POLICY FOR NAME ON DIPLOMA

Please read the following instructions carefully BEFORE completing the next section.

The name on your diploma must be the same as it is in the university Student Information System (SIS). However, any portion or derivative of that name will be acceptable. For example, if your name in SIS is Mary Ann Andrews Jones it would be acceptable for the diploma to read:

Mary Ann Andrews Jones
Mary A. Jones
M. A. Jones
Mary Jones
However, the following names would not be acceptable:
Mary Ann Jones Smith (new married name)
Mary Ann Andrews (maiden name)
A degree candidate may change his/her name in the Registrar's Office up to one term following graduation. Specific policies and procedures must be followed as outlined by the Registrar's Office.

Include the full name of the institution from which you earned your degree(s) as well as the entire name of your degree, e.g., Bachelor of Arts, Bachelor of Engineering, Bachelor of Science, Master of Theology, Master of Public Administration, Bachelor of Business Administration, Bachelor of Professional Studies, Bachelor of Pharmacy, etc.

Do not include the name of your "academic major" unless it is part of your degree designation, e.g., Bachelor of Science in Education.
