DIRECTIONS FOR COMPLETING SPECIAL TOPICS FORM

I. Enter DEPARTMENT name

II. Pull down TERM and YEAR that Special Topics will be taught.

III. Complete all boxes in the COURSE MASTER IDENTIFICATION section.
· Pull down for choices of INSTRUCTIONAL METHOD.
· Enter grading other than A-F in special grading section.
· Enter dates other than regular semester calendar in ABBREVIATED SCHEDULE.

IV. After approvals from the Department and College are completed and a course syllabus is attached, submit form to Academic Affairs, Administration Building, Room 217.

V. Submit name and e-mail as a contact source.

After all data have been entered into the form, use your browser's print command to print the form for submission to the appropriate committee for review. If you are a Mac user, use Internet Explorer as your browser in order to print the form after you have filled it out (Netscape only allows you to print out the form itself). After filling out and printing the form, click the reset button to create another course request.
